NOTE: In order to download the form, use the "download arrow button" in the top right corner of the registration form to the right. It's next to the "print" button option in the top right corner. If you have any questions, email us. You have to scroll down on the document below to see all 3 pages for payment/scholarship donation/registration/t-shirt or hoodie order instructions. OPTION 1: If you are mailing in your payment via the PO box address on the form. All you have to do is download the form, print/fill it out, and send payment with registration form to PO box address on the form. OPTION 2: If you would like to pay via electronic invoice (via Square). Please send an E-mail with a completed Registration form (including T-shirt/Hoodie Orders, Scholarship Donations, and Payment Schedule) to [email protected]. You will receive a Square Invoice via email with further payment instructions for the payment amounts and dates. |
